Requirements to apply

• Principal - Above 21 years of age.
• Supplementary - 18 years of age.
• Classic card, annual income RM18,000 which means 1500 per month minimum.
• Gold card RM40,000 annually which means around RM3500 per month minimum.

Basic documents to apply

• Photocopy of IC (Front and back)
• Photocopy of passport (Foreigners only)
• Completed and signed application form.
• Temporary IC must be supported by copy of driving license or passport.

Other supporting documents needed

• 3 months pay slip or bank statement showing salary credited or latest EA form or Latest EPF stament or latest form BE with tax receipt or Letter of appointment (if employed less than 3 months)
• For self employed parties you can submit your latest form b with tax payment receipt and business registration form (BRF)***
